• Declaration of Immigration Status -- This form must now be completed by all non-U.S. citizens, including U.S. permanent residents.  You may bring it with you to the KITP or fax it to the number below prior to your arrival.
• J-1 process initiation form (4 questions) -- The J-1 application procedure has changed. If you wish to apply for a J-1 visa, please complete the above form and either fax it to the KITP or email it as a pdf with the required attachments to Timber Kelley. Once she has this information, she will request a username and password be sent to you from the SEVIS system. You will then be able to complete the J-visa online application which will initiate the processing here at UCSB of the DS-2019 form needed for the J-visa.
