The Family Fund questionnaire is not quite ready yet, but we want to make this resource available to you as quickly as possible.

Until we have the questionnaire up and running, you may apply for the Family Fund by sending us an email that provides the following information*:

1) Your first and last name;

2) The name of the program you will be attending;

3) The dates you will be here at the KITP;

4) Will your spouse/partner visit?    If yes, the dates of the spouse/partner visit;

5) Will children visit?  If yes, the dates of the chidren's visit, plus the names and ages of the children;

6) Please write to us and briefly explain how assistance from the Family Fund will help facilitate your visit

7) Please indicate whether you could rent your home or apply other income to offset some of the expense of your coming here, and if so, approximately how much you might receive.

8) If you are not a U.S. citizen or Permanent Resident, please provide your visa status.  This will help us to determine what form the award may take.

Please note, we cannot commit these funds if you have not received an official invitation to the program.

Your response will be seen only by relevant KITP staffers; however, if you feel that your request contains confidential information and prefer to discuss your situation with one person only, please call or email Deborah.

We will try to respond to your request as quickly as possible. As there is some codependency built into the situation (our needing to know how much housing costs in order to make a determination while a scientist needs to know about the extra funding before committing to housing) some time for additional correspondence and information gathering may be required.
