If you are departing soon please take a moment to click on the link below and upload your activity report before you leave the KITP. The upload accepts several formats, so there will certainly be one that is right for you. This should work for all mainstream browsers, but you do need to have cookies enabled.
While there, we ask that you also take a moment to complete our new exit surveys. These are very simple and should only take a few minutes of your time. The data they provide is very useful for us, both to satisfy requests from NSF and to provide better service to the community.
If there is not a link here for your activity, please select GM_YEAR. If you should have any difficulty uploading your report you can still send it to reports. Also, if you need any KITP report (preprint) numbers now, please indicate it in your report (it helps us to find them and process them more quickly). You can also request KITP report numbers at any time directly from preprints. We often receive funds from other agencies and foundations in support of our activities. Please check the webpage for your program to be sure that you acknowledge this support along with that provided by the NSF.

Dear Participants,
Your program has ended/is ending soon and, according to our records, we have not received your activity report.
Your program coordinators will be writing their final report for submission to the NSF soon and would like to have your feedback to prepare that report. Please take a few moments to click on the link below, complete the short survey and upload your activity report. Your activity report should include a general description of your work here, KITP reports and papers for publication---in preparation or completed---and names of chief collaborators. It would also be appreciated and helpful to us if you would mention in your activity report future plans related to your stay at the Institute.
Please be sure to email preprints your requests for KITP report numbers for all papers completed before as well as after your departure so that they may be prepared as KITP reports. (Please be sure to include the title of your paper and names of co-authors in your request). Please Note: These works constitute an essential record of research done here.
